Introduction. 2. objectives and definition. 3. the description of an adverse effect or near miss and the contributing factors. 4. Potential or actual financial costs involved with adverse event. 5. Expected actions and accountability of the staff involved. 6. Root Cause Analysis need to used the method to analyse the adverse event. 7. Discuss the significance of – communication, the hospital environment, latent factors, equipment, system barriers, rules, policies, procedures, fatigue, rosters. 8.
