You will demonstrate the criterion of this assessment in a simulated environment at the EQUALScampus You are required to work with 3 different people with mental illness. For the purpose of thisassessment, the people you will be working with will be 3 of your classmates. You are to assume the role of the service provider within the Mental Health industry and as suchshould ensure you have an understanding of wellness plans prior to demonstrating the requirementsof this assessment. You will be required to familiarise yourself with the ACD (Advance Care Directive) Guide that isavailable for download on Moodle to ensure you are able to effectively provide information to theperson in Part 3 of this assessment. Demonstration should occur over 3 different times to allow for adequate planning and assessment. The person playing the role of the person with mental illness should be allowed adequate time toconsider their role, what their background may be, what wellness means to them and the goals theymay want to include in their wellness plan. They are able to embellish the role as much as they like aslong as it fits in with the context of the assessment task. You are required to record the relevant information from part 2 of this assessment on the wellnessplan and Part 3 in the advance care directive template and submit for final assessment. Templates ofthese documents are available for download on Moodle. You may be allocated additional time tocomplete the Wellness Plan and Advanced Care directive. Where required, this may be completedoutside of class time. You will be provided with this assessment in adequate time to allow for preparation of your roles andwill be allocated time during your sessions for this unit to prepare for your simulation. The time allowed for the simulation/scenario activity assessment is 60 minutes. This is a guide onlyand you may be allowed more or less time to complete the assessment tasks. Completed wellness plan (1 for each person) that addresses the criterion identified in part 2 of thisassessment Completed Advance Care Directive Form (using template available on Moodle) for each person.Assessment Task: SimulationFor this assessment, you are required to assume the role of a service provider working in a mental healthorganisation.Your role is to work collaboratively with the person experiencing mental illness to develop a wellness plan thatwill support their recovery as well as an Advance Care Directive.You will be required to demonstrate the criteria identified in each of the four parts for this assessment:Part 1 – Collect informationYou will meet with the person with mental illness and discuss wellness planning, you will be required to:• Provide information in relation to wellness planning including what it is, how it can help them and whatthey may include in the plan – consider using relevant published information including websites or printmaterial.• Discuss what differences the person would like to see in their life and the values they would like touphold in relation to wellness planning.• Work collaboratively with the person to identify if there are any cultural factors to consider that mayinfluence the wellness plan, this may be in relation to religion, cultural background, gender, sexualpreferences etc.• Work collaboratively with the person to determine who may need to be consulted regarding the wellnessplan (family, friends, medical practitioners, other professionals)• Provide the person with other information or documents that may be relevant to their situation. This maybe in relation to their support options, forms they may require to access health professionals etc.Part 2 – Establish the Wellness PlanWorking collaboratively with the person, you will be required to decide on the type of wellness plan to becompleted and the items and areas to be included. You are meeting with them today as you had a telephone call with them earlier andyou were concerned with their current mental state. During your meeting you will need to:• Provide and maintain a positive and emotionally safe environment for the person to enter• Sensitively and supportively respond to the person in distress• Collaboratively discuss the nature of distress and implement wellness strategies in conjunction withtheir wellness plan• Discuss the need for emergency assistance with the person to determine if others are required to beinvolved• Record the details of the incident in the person’s file and adjust wellness plan as required to includeadditional strategies implemented during this meeting.• Arrange a follow up appointment with the person in the near future to assess and discuss continuingwellness strategies.Student Case Studies:Student to use the 3 Case Studies and construct a Wellness Plan for each. The person in the case studywould be discussing these issues with the student and that they were working together to build a plan. After aplan has been constructed, the student is required to answer the following questions.1. Jason likes to do his own banking and shopping and can address most of his needs.Jason admits when he is unsure or needs more information.Jason has had a history of alcohol and drug abuse. When he had done this in the past, he was living in hisown house. Jason’s social circle was considered to be toxic as the only people he socialised with, all useddrugs. They would all meet at Jason’s house and as time when on, the house became damaged andunhygienic due to not being cleaned.
